Transition planning is key for students with disability to get needed services and supports while still in high school and looking to engage in adult life. What is needed now and for tomorrow for the student to be success? Everyone has a role in the process and each is as important as the other. Understanding those roles and developing strategies around the students transition goals helps ensure a smoother transition.
